Perpetual Mass Enrollment
Use this online form to enroll your loved ones—living or deceased—with a Perpetual Enrollment in our Mass Association. This online form processes your donation using PayPal. You don’t need a PayPal account to use Pay Pal. Once you have enrolled your loved ones we will ship the enrollment folder to the address you provide. Please allow two weeks for delivery.
